Please read each statement carefully. You will be required to initial or sign at the time of your appointment stating that you have read and agree to the terms.
All sessions are done virtually at this time. It is the clients responsibility to ensure they are in a quiet and private location for the time of the visit. A visit will not be conducted if the client is driving or engaging in other distracting activities. If the client is unable to complete the visit due to the reasons stated above, they will be charged for the entire visit time.
All information disclosed within sessions is confidential. We will exchange information with your team of professionals ONLY by signing the Authorization to Release Information Form.
All services are to be paid at the time of the appointment. Fees as of July1st, 2022 are as follows:
Initial appointment (60 minutes) - $175
Follow up (30 minutes) - $100
Premium Package - $750
Please note that fees are subject to increase. Clients will be given at least one month notice of increase.
As of July 1st, 2022, Bee Well Nutrition, LLC is currently an out-of-network insurance provider. You may request a Superbill which you may submit to insurance for reimbursement purposes. A Superbill does not guarantee payment for services. It is the responsibility of the client to check insurance benefits and communicate with the insurance provider about any type of reimbursement you may receive.
Bee Well Nutrition, LLC does not provide refunds for services.
Any person under the age of 18 years should have an parent/legal guardian involved in the nutrition counseling process.
A new law was passed that requires out of network providers to estimate direct costs. You will be given an individualized "Good Faith Estimate" of treatment for the next year. This will be included in your form packet.
You are not required to obtain the number of visits in the "Good Faith Estimate". It is an estimate of cost of services should they be provided to you for an entire year.
A 48 hour notice is required to cancel/reschedule an appointment. Failure to cancel within 48 hours will require full payment for the appointment.
Appointment times will be held for 15 minutes. After 15 minutes from the appointment time, you will be charged a late fee $50 and asked to reschedule your appointment.
Two attempts will be made to reach the client at their designated appointment time.
First attempt: at the appointment time
Second attempt: 10 minutes after the appointment time
If the client does not answer at either attempt made for the visit, they will be charged for the entirety of their visit.
